Step 1: you tell us what is hard now
Stepping over the tub wall, a shower you no longer trust standing in, a doorway that has become difficult, a ramp needed at the front door, or a fall that has already happened - plus roughly how old the home is and which part of the county you are in. If a veteran is in the household, say so: it changes the questions worth asking, and the programmes themselves are explained on va.gov rather than here.
You do not need to know which modification you want, and you do not need a diagnosis or anyone's assessment to send the form. Working out what the room needs is what the contractor does on the visit, and it is not a decision this site makes or can make.
Step 2: we introduce a licensed contractor who works your part of Collier County

That written quote is the only figure that applies to your home - the published ranges on this site describe the market, not your bathroom. Do you assess the home or decide what is needed?
No. Nothing here inspects a home or produces an assessment. You describe what is hard now; the contractor who visits looks at the bathroom, the doorway, the floor structure and the door, and works out what the room needs. Published ranges tell you which order of magnitude you are in - they do not decide your scope.
How long does the whole thing take?
The schedule is the contractor's, and the honest answer for your address is theirs rather than a figure published here. What you can insist on is that any schedule is in writing, and that the contractor tells you what could move it - permit timing, an HOA application, or what is found once the wall or floor is open.
